AYURKALP NOTICE OF PRIVACY PRACTICES
By availing the service of onphone/online ayurvedic consultation through Ayurkalp, you acknowledge and agree that,
- Ayurvedic advise such provided is not a replacement or substitute for a direct face to face consultation with the Doctor. It only supplements it to enable you to make a well informed choice.
- The opinion is subject to the limitations described above.
- The data, information and reports provided onphone / online may have limitations of quality and may be bereft of required information and the ayurvedic expert may seek more and clear information / reports without which he may not be in a position to perform his professional duty and may refuse to give a comprehensive opinion or answer the query, in the absence of that additional information.
- In cases where the ayurvedic expert advises that there is a necessity of physical examination to arrive at a just advice or discuss with the medical expert who has physically examined the patient, he may refuse to provide the advise, in the absence of that.
- The Ayurkalp service is totally based on the information and material provided by the person seeking the Ayurvedic advice or treatment and hence any information demonstrated to have been falsified or concealed, will immediately render the advise and all details therein, null and void.
- Delays in delivering medicines at your doorstep could occur due to deficiencies or failures of the equipment or reasons beyond our control. Ayurkalp shall not be held liable for such delays.
- In very rare instances, security protocols could fail, causing a breach of privacy of personal medical information even though all reasonable efforts & precautions are routinely taken. Ayurkalp shall not be held liable for them.
- In rare cases, a lack of access to complete medical records may result in adverse drug interactions or allergic reactions or other judgment errors. Ayurkalp doctors shall not be held liable for them.
By agreeing to avail the services of Ayurkalp, you expressly acknowledge that you had been informed in detail and understand & agree specifically with the following:
- You understand that Ayurkalp is following strict privacy policies and is governed by statutory regulations that protect privacy and the confidentiality of medical information. No information obtained in the course of the services offered through Ayurkalp, which identifies the patient, will be disclosed to researchers or other entities without your consent.
- You understand that the services of ayurvedic consultation is in addition and not in exclusion to the regular consultation and in choosing to participate, you understand that this does not replace the direct opinion obtained by face to face consultation and should be considered as of an advisory nature and to be verified and validated by way of personal interaction with your physician.
- You understand that the consultation may involve electronic / telephonic communication of your personal medical information to other medical practitioners who may be located in other areas, including out of state or country, after discussing with you.
- You understand that you may expect the anticipated benefits from the use of the Ayurkalp in your care, but that no results of treatment can be guaranteed or assured.
- Advise(s) provided has been provided on the basis of the information, material and reports provided by you and it will not be and cannot be used as a medico legal document.
- Ayurkalp accepts no liability for the contents of the advise given by the doctor, which is solely the opinion of the expert, given at the instance of the patient. Ayurkalp is also not liable whether or not that advise is accepted or used by the patient or his physician or any other physician.
- All advises from Ayurkalp are valid for 1 (one) days following the date on which the advice is given by the doctor. Ayurkalp accepts no liability for the advise or its follow up queries, if any, and after expiry of this period, no queries on the issue will be entertained. For any queries after this 1(one) day time limit, the system will have to register afresh with fresh payment. The advises & responses to the queries, if any, presented on this platform are solely those of the author (doctor) and do not necessarily represent those of the service provider (Ayurkalp).
